Mistovate 0.5mg

This product, marketed by Healthgate, is Fluticasone Propionate Respirator Suspension, branded as MISTOVATE. The medication is presented in a white box with curved dark purple and maroon band accents, featuring multi-colored ring graphics. The packaging indicates that it contains 4 strips of 5 single-use ampoules (respules), each holding 2 ml of solution (a total of 20 respules). A sealed foil pouch in the foreground contains a single strip of 5 respules. The label specifies “For inhalation only” and “Preservative Free.”

Product Composition

This medication is an inhalation suspension containing a single active ingredient:

  • Fluticasone Propionate (0.5 mg): Expressed as 0.5 mg of fluticasone propionate per 2 ml respule. Fluticasone Propionate is a synthetic, highly potent, halogenated corticosteroid. It has powerful anti-inflammatory effects and works by suppressing the inflammation process in the airways, reducing swelling and irritation.

Summary

MISTOVATE is a prescription-only inhalation suspension containing the corticosteroid Fluticasone Propionate. It is strictly for long-term (maintenance) treatment to prevent breathing difficulties and exacerbations in patients with persistent asthma and certain types of chronic obstructive pulmonary disease (COPD). By reducing airway inflammation over time, it helps to open the airways and improve lung function. This medication is not intended for the rapid relief of acute bronchospasm (as a “rescue” inhaler).

Pros and Cons

Pros

  • Highly Potent Anti-inflammatory: Provides robust control over the underlying airway inflammation that causes respiratory distress.

  • Preventative Treatment: When used consistently, significantly reduces the frequency and severity of asthma attacks and COPD flare-ups.

  • Convenient Nebulisation Form: Ideal for patients who have difficulty effectively using a standard metered-dose inhaler (MDI), such as children, the elderly, or those with severe respiratory distress.

  • Preservative-Free: May be better tolerated by patients who have sensitivities to preservatives.

Cons

  • Requires Equipment: Demands the use of a nebuliser machine, making it less portable and more time-consuming than using an MDI.

  • Not for Quick Relief: Takes days to weeks to reach full effectiveness; not for managing a sudden, acute attack.

  • Potential for Oral Infections: Common side effects include thrush (candidiasis) in the mouth or throat, and throat irritation or hoarseness.

  • Systemic Side Effects (High Doses/Long-Term): High doses or prolonged use can, in rare cases, lead to systemic corticosteroid effects like bone density loss or cataract formation.

  • Interacts with certain medicines: Can interact with certain antifungal or antiviral medications.

How to Use

This medication is available by prescription only and must be used with a nebuliser machine strictly under medical supervision. Below is a general guide. Follow your doctor’s specific instructions and the nebuliser manufacturer’s guidelines precisely.

  1. Preparation: Wash your hands thoroughly. Prepare your nebuliser machine for use according to its manual.

  2. Open the Ampoule (Respule): Twist off the top of one (1) single-use MISTOVATE ampoule.

  3. Transfer the Medicine: Squeeze the entire contents of the single ampoule into the nebuliser medication cup. Do not mix this medication with any other medicines in the cup unless directed by your doctor.

  4. Administer the Treatment: Assemble the nebuliser cup with the mouthpiece or face mask. Turn on the nebuliser machine. Breathe in and out slowly and deeply through your mouth until all the medication mist is gone (typically 10-15 minutes).

  5. Post-Treatment: After the treatment is complete, thoroughly rinse your mouth with water and spit it out. Do not swallow the rinse water. This crucial step helps prevent the development of oral thrush. Wash your face if you used a mask. Clean the nebuliser components.

  6. Discard Used Ampoule: Safely discard the empty single-use ampoule. Use a fresh ampoule for each treatment. Do not save any remaining solution.

  7. Course of Treatment: Complete the full course of treatment as prescribed, typically twice daily (every 12 hours), even if you feel better.

  8. Inform Your Doctor: Tell your doctor about any pre-existing medical conditions, especially severe liver disease, diabetes, glaucoma, or a history of tuberculosis. Inform them of all other medications you are taking, especially other steroids.

  9. Seek Medical Help Immediately: Contact your healthcare provider at once if your breathing difficulties get worse during or after a treatment, if you notice symptoms of a mouth infection (white patches in mouth/throat, pain), or if you develop chest tightness or a rapid/irregular heartbeat. This is not a “rescue” inhaler. A separate rescue inhaler should always be available for sudden attacks.
